New Therapies for COVID19 (Compiled from various sources by Anoop Misra with inputs from Rahul Pandit):
Oral Drugs
1. Paxlovid (Pfizer): The FDA cleared the pill for patients 12 and up with mild to moderate Covid who are most likely to end up hospitalized or not survive (30-89% effective in preventing hospitalisation). The agency said it should be prescribed as soon as possible after diagnosis and within five days of symptom onset. Pfizer’s treatment is administered in two 150 milligram tablets along with a 100-milligram tablet of an HIV drug, ritonavir, twice daily. The HIV drug helps slow the patient’s metabolism, which allows Paxlovid to remain active in the body at a higher concentration for a longer period. According to Pfizer, laboratory studies show that it is likely to remain effective against Omicron.
2. Molnupravir (Merck): Causes mutation errors in replicating virus. Reduces hospitalisation and deaths by 30%. Approved by EU. Merck’s 800 milligram pill is taken every 12 hours for five days after symptom onset. Emergence of variants remains a concern.
Monoclonal Antibodies
1. Both Regeneron and Lilly’s antibodies did a poor job of blocking the variant virus from invading cells. Both Regeneron and Eli Lilly say they are developing monoclonal antibodies for Omicron, but it will be months before they are ready for use.
2. Sotrovimab (GSK), by contrast, remained potent. The antibody binds to an epitope on SARS-CoV-2 shared with SARS-CoV-1 (the virus that causes SARS), indicating that the epitope is highly conserved, which may make it more difficult for resistance to develop with any new variants.FDA has given emergency use authorisation to this drug.
