In reply from Gautam Gambhir Foundation they shared a link https://forms.gle/c7iaui56iaFX5WPg8, and asked us to fill the form with details like- Email id, Name of the patients, age, photo of covid positive report and adhaar card details etc. Following the submission of form his team will reply to those who are in need of the concentrator.
